Though we're always cautious about recommending nicotine products, it's hard to deny that they're less dangerous than cigarette smoking.
The term "vaping" comes from the act of inhaling and exhaling e-cigarette vapor (which is actually an aerosol). E-cigarettes heat up liquid from replaceable cartridges, producing the vapor. Just like a smoker does, an e-cigarette user inhales the vapor through the device, then exhales. With regards to toxicity, little is known about the effects of humectants when they are heated and chronically inhaled. Indeed, the latter study established that one e-cigarette puff results in a PG exposure of 430–603 mg/m3, which is higher than the levels reported to cause airway irritation (average 309 mg/m3) based on a human study [55]. The same study established that one e-cigarette puff results in a glycerol exposure of 348–495 mg/m3 [57], which is close to the levels reported to cause airway irritation in rats (662 mg/m3) [58]. Results from in vitro studies are in general agreement with the limited number of in vivo studies.
Much like free-base nicotine products, these liquids contain propylene glycol and/or vegetable glycerin, as well as other chemicals such as flavouring ingredients. Most vaping devices use electrical power from a battery to heat a liquid solution. The term "vaping" is also used to refer to the inhalation of cannabis aerosols, which are produced by products that differ from nicotine products described on this page. Propylene glycol and glycerin are the most common solvents used in vapor products. While generally recognized as safe for ingestion by the FDA, research has found both substances to be toxic when aerosolized through the vaping process.
Silica was commonly the first material to be used as a wick, followed by cotton and ceramic3,13,14,15. Cotton has good wicking properties but is less thermally stable than silica14,16,17, while ceramic is chemically stable and heat-resistant18. The use of microporous ceramic as a wicking material has increased in the past few years14,16,18,19,20. Additional research is ongoing to clarify the dangers from vaping, and how vaping damages the lungs and other organs. Such policies are critical in the absence of strong federal regulation of e-cigarettes. As of March 31, 2023, seven states and an estimated 388 counties, cities, towns, and tribes have restricted the sale of flavored tobacco products. However, this leaves a large majority of the U.S. population not covered by such restrictions. E-cigarette sales have rapidly escalated in the relatively short history of the products. Overall e-cigarette sales, as well as the total number of e-cigarette brands, increased by more than 46% between January 2020 and December 2022. Disposable e-cigarettes have nearly tripled in nicotine strength, quintupled in e-liquid capacity, and dropped in price by nearly 70% between 2017 and 2022.
WASHINGTON (AP) — The Supreme Court took up an e-cigarette case Tuesday, weighing Food and Drug Administration decisions blocking the marketing of sweet flavored products amid a surge in vaping by young people. Despite not authorizing a single synthetic nicotine product, it has allowed these products – including e-cigarettes in kid-friendly flavors – to stay on the market well past the July 13 deadline. There is little research on the safety of vaping in pregnancy, but it's likely to be substantially less harmful for you and your baby than smoking. Unlike cigarettes, vapes do not produce carbon monoxide, which is very harmful to a developing baby in the womb. They also cause other serious illnesses, including lung disease, heart disease and stroke.
CATCH My Breath’s curriculum attempts to empower students with this information for them to decide whether to pick up an e-cigarette, Marcella Bianco, the program’s director of government partnerships, explained. Once the school year starts, the 34 middle and high schools in the district join 56 others across the state that have already implemented the prevention program since its launch in 2017. The participants were encouraged to set a quit date and were asked about their vaping behavior via follow-up text message assessments. The trial participants were recruited via ads on social media, including Instagram, Facebook and Snapchat. From October 2021 to October 2023, about half of the participants were randomly assigned to the This is Quitting text message program.
The Mountain state has fewer restrictions than Wyoming and allows the sale of e-cigarettes to people under 21. Researchers from online prescription marketplace Universal Drugstore, analyzed federal and state health data on air quality, youth obesity, sports participation, mental health, test scores, and youth e-cigarette use. Based on a 2023 survey, the team found Wyoming had the highest rate of youth vaping, with nearly one in three teens using e-cigarettes in the last year, despite the state's no-sale laws for residents under 21. Parental smoking, positive attitude towards e-cigarette use in one’s age group, current smoking, and snus use increased S-EC, whereas liking school lowered it. At school level, both among girls and boys, a higher proportion of students liking school and being never-users lowered S-EC, whereas a higher proportion of students with positive attitudes towards e-cigarette use increased it. Among girls, a higher proportion of girls in the school, students with a high parental education and planning for GUSS lowered S-EC, but a higher proportion of students with parental smoking increased it.
Almost 9 in 10 people (86%) supported prohibiting the sale of e‑cigarettes to people under 18 years of age, a substantial increase from 2019 (79%). Social and economic factors shape people’s behaviours of vaping or smoking. Generally, people living in the most disadvantaged socioeconomic areas were the most likely to currently smoke regular cigarettes but not use e‑cigarettes (13.2%). In contrast, people living in the most advantaged socioeconomic areas were the most likely to use e‑cigarettes but not smoke regular cigarettes (6.6%) (Figure 4). Young people aged 18 to 24 were the most likely to have used e‑cigarettes in 2022–2023, with almost 1 in 2 (49%) having used them at least once in their lifetime, and over 1 in 5 (21%) currently using e‑cigarettes in 2022–2023. In many high and middle income countries lower socioeconomic status is a strong predictor of smoking. In North America, the common name for the remains of a cigarette after smoking is a cigarette butt. In Britain, it is also called a fag-end or a dog-end.[44] The butt is typically about 30% of the cigarette's original length. It consists of a tissue tube which holds a filter and some remains of tobacco mixed with ash. According to data from the World Health Organization,[39] the amount of tobacco per 1000 cigarettes fell from 1.03 kg (2.28 pounds) in 1960 to 0.41 kg (0.91 pounds) in 1999, largely as a result of reconstituting tobacco, fluffing, and additives. The epidemic use of e-cigarettes is a complicated problem which requires a cooperative effort between partners, organizations, and communities.
The FDA does not currently require testing of all the substances in e-cigarettes to ensure they are safe. It's also hard to know exactly what chemicals are in an e-cigarette because most products do not list all of the harmful or potentially harmful substances contained in them. E-cigarettes do not contain tobacco, but many of them contain nicotine, which comes from tobacco. Because of this, the Food and Drug Administration (FDA) classifies them as "tobacco products." For adults who smoke, switching completely from cigarettes to e-cigarettes may reduce exposure to many harmful chemicals present in cigarettes. In the multivariate model (Table 3), results were similar for both girls and boys. S-EC remained higher for those planning for vocational education, and lower with liking school. Further, S-EC was higher with parental smoking, positive attitudes towards e-cigarette use in one’s age group, current smoking, and current snus use. The decision to pursue a smoking cessation objective, even in such a controlled form, should be made only after considering national circumstances, along with the risk of uptake and after exhausting other proven cessation strategies. Vaping has exploded into a national crisis, and tobacco companies are helping to fuel it by targeting kids with flavors such as gummy bear and cotton candy. E-cigarette companies have promoted unsubstantiated health claims about their products as healthier than traditional cigarettes, when, in fact, e-cigarettes are uniquely dangerous for kids due to nicotine’s impact on their developing brains. Since being introduced to the U.S. market in 2007, e-cigarette use among youth has increased to epidemic levels (Surgeon General's Advisory on E-cigarette Use Among Youth, 2018). The availability of flavored e-cigarettes is one of the most commonly cited reasons for e-cigarette use among youth (Tsai et al., 2018). Given the importance of flavors contributing to use of e-cigarettes among youth, these data briefs build on this previously published article, providing an update on trends in unit sales of e-cigarettes in the U.S. by product and flavor type.
"From a public health perspective, we have always been concerned about dual-use of both traditional and e-cig products," said study lead author Marisa Bittoni, an oncology researcher at Ohio State University in Columbus. A second brand of e-cigarettes — marketed as Nixotine, Nixodine, Nixamide and Nic-Safe – contained a nicotine analog called nicotinamide, also at levels lower than the labels indicated, and combined with undisclosed amounts of 6-methyl nicotine. He emphasised that e-cigarettes are not an effective tobacco cessation tool, as they are not approved by food regulatory bodies. Additionally, they often lead to dual usage with people continuing to use traditional cigarettes along e-cigarettes.
Most of the harmful chemicals in tobacco smoke, including tar and carbon monoxide, are not contained in vape aerosol. Switching to vaping significantly reduces your exposure to toxins that can cause cancer, lung disease, and diseases of the heart and circulation like heart attack and stroke. These diseases are not caused by nicotine, which is relatively harmless to health.
It can’t hurt to talk with a doctor or other healthcare professional about the risks of vaping, especially if you already have a chronic health condition, such as asthma. More research needs to be done in order to understand the side effects of nicotine-free vaping. Some of these cellular changes have been linked to the development of cancer over the long term, though there’s currently no evidence to suggest that vaping causes cancer. The 2018 NAP report found substantial evidence that vaping causes cell dysfunction, oxidative stress, and damage to DNA.
Vape aerosols may also increase the risk of heart disease, lung cancer, and asthma complications. The substitution was especially evident among cigarette brands popular with young people aged 20 and under, suggesting that flavor restrictions may increase smoking among youth as well as adults. The immense popularity of electronic cigarettes, or e-cigarettes, among young people has led many policymakers to restrict the sale of flavored varieties. Proponents of e-cigarettes claim they’re safer than smoking because they don’t contain the more than 60 cancer-causing chemicals in tobacco smoke and are not combustible. But e-cigs still deliver harmful chemicals, including nicotine, the extremely addictive substance in cigarettes.
And nicotine use in young adults still can lead to other illicit substance use. The request from the Federal Trade Commission asks six companies to turn over data by January dealing with the sale and promotion of their products for the years 2015 to 2018. In the United States, states are a primary determinant of the total tax rate on cigarettes. As part of the Family Smoking Prevention and Tobacco Control Act, the federal government collects user fees to fund Food and Drug Administration (FDA) regulatory measures over tobacco.
Some people believe that e-cigarettes can help them quit smoking tobacco. However, nicotine patches and many other FDA-approved quit aids are available now to help people quit smoking. E-cigarettes are not currently approved by the FDA as aids to help stop smoking.

Of this sample, 3,242 participants developed heart failure within a median follow-up time of 45 months. As e-cigarettes deliver flavorings, additives, and, typically, nicotine through an inhaled aerosol, they are harmful to both the user and those who are exposed secondhand.
In a major recent undertaking to help clarify the effects of e-cigarettes, the physician-investigator conducted a review of the best available evidence about vaping and lung harms. She and her co-authors published their roundup of the evidence in an article titled "Vaping and Lung Inflammation and Injury," which appeared online Feb. 10 in the journal Annual Review of Physiology. Many e-cigarettes sold in the U.S. contain far more nicotine than e-cigarettes sold elsewhere, which increases the risk of addiction and harm to the developing brains of youth and young adults. Marketing tactics targeting young people have contributed to the rapid increase in use. The long-term risks of exclusive use of e-cigarettes are not fully known but evidence is accumulating that e-cigarette use has negative effects on the cardiovascular system and lungs.

The FDA must act without further delay to remove all flavored e-cigarettes from the market. CDC recommends you not use e-cigarettes or vapor products, especially those with THC. Fewer Pierce County youth are smoking cigarettes in recent years, but more are vaping. The main ingredient in vapes is propylene glycol, vegetable glycerine or glycerol, and almost all vapes contain nicotine, flavours and other chemicals.
E-cigarettes are not safe for youth, young adults, and pregnant women, and can be dangerous for adults who use tobacco products. The novel results for school-level factors indicate that prevention of nicotine use may be supported by facilitating nicotine-free schools through policies and programmes. This may address especially the peer selection processes, which have been observed both in school disengagement and smoking initiation [28, 29]. Further, it may influence the students’ descriptive and injunctive norms, which both predict smoking initiation among youth [27].
An assessment from the agency, issued in 2008, references only a couple of studies that cover inhalation exposures—all with laboratory animals rather than people. The minimum age to purchase e-cigarettes and other tobacco products has also been raised to 21. A bill was also introduced in 2022 to raise the minimum age to purchase tobacco products and vapes from 19 to 21, but it was vetoed by Governor Mike Dunleavy.
Most modern cigarettes are filtered, although this does not make the smoke inhaled from them contain fewer carcinogens and harmful chemicals. But when it comes to your heart health, a pair of recent studies show e-cigarettes are just as dangerous – and possibly are even more dangerous – than traditional cigarettes. The program’s curriculum includes a four-part series of lessons, with one set for fifth through eighth graders and another for high schoolers. When studying tobacco cigarettes, researchers rely on smoking machines that simulate how frequently a typical smoker takes a puff and how much smoke is inhaled with each breath. No one has yet determined how much e-cig vapor the typical user breathes in, so different studies assume different amounts of vapor as their standard, making it difficult to compare their results. Tracing what happens to that vapor once it is inhaled is equally problematic.
