So far, no vaping products have been licensed as stop smoking medicines in the UK, so they are not available on prescription from the NHS or from a GP.

The FDA has the legal authority to regulate tobacco products containing nicotine from any source, including synthetic nicotine. In 2016, the FDA established a rule for e-cigarettes and their liquid solutions. Because e-cigarettes contain nicotine derived from tobacco, they are now subject to government regulation as tobacco products.
Recent guidance from the Center for Tobacco Products indicates the importance of switching completely away from combustible cigarettes for those who are also using e-cigarettes," she added. The investigators revealed that daily vs nondaily use of e-cigarettes was linked to higher overall rates of quitting combustible cigarettes (12.8% vs 6.1%). The adults who used e-cigarettes in 2019 were more likely to stop smoking traditional cigarettes compared with those who used the e-cigarettes between 2014 and 2015 (12.0% vs 5.3%). In 2022–2023, 2.3% of people in Australia both smoked regular cigarettes and used e‑cigarettes, an increase from 0.7% in 2016. In the second study, researchers analyzed heart blood flow, a measure of coronary vascular function, of 19 young adult smokers immediately before and after smoking either e-cigarettes or tobacco cigarettes. They examined coronary vascular function by a myocardial contrast echocardiography while participants were at rest and after performing a handgrip exercise to simulate physiologic stress. In the first study, researchers looked at the impact of e-cigarettes on lipids and glucose in the blood. They recruited 476 healthy human participants without cardiovascular disease who were either nonsmokers, e-cigarette-only smokers, smokers of e-cigarettes and tobacco cigarettes and those who smoked tobacco cigarettes only.

In this latter regard, no differences were encountered in the frequency of tumour appearance in rats subjected to long-term (2 years) inhalation of nicotine when compared with control rats [48]. Despite the lack of carcinogenicity evidence, it has been reported that nicotine promotes tumour cell survival by decreasing apoptosis and increasing proliferation [49], indicating that it may work as a "tumour enhancer". In a very recent study, chronic administration of nicotine to mice (1 mg/kg every 3 days for a 60-day period) enhanced brain metastasis by skewing the polarity of M2 microglia, which increases metastatic tumour growth [50]. Assuming that a conventional cigarette contains 0.172–1.702 mg of nicotine [51], the daily nicotine dose administered to these animals corresponds to 40–400 cigarettes for a 70 kg-adult, which is a dose of an extremely heavy smoker.

E-cigarettes (often called vape pens) are battery-operated vaping devices that heat a liquid until it becomes an aerosol (mist), which is inhaled. It usually contains nicotine, other harmful chemicals, and flavorings. Even e-cigarettes that claim to have no nicotine have been found to contain nicotine. A non-smoker who uses ENDS may become addicted to nicotine and find it difficult to stop using ENDS or become addicted to conventional tobacco products. Further, many e-cigarettes are designed to allow the control of nicotine (which can reach dangerously high levels).

Nevertheless, the safety of e-cigarette consumption and its potential as a smoking cessation method remain controversial due to limited evidence. Moreover, it has been reported that the heating process itself can lead to the formation of new decomposition compounds of questionable toxicity. Numerous in vivo and in vitro studies have been performed to better understand the impact of these new inhalable compounds on human health.
Some people believe that e-cigarettes can help them quit smoking tobacco. However, nicotine patches and many other FDA-approved quit aids are available now to help people quit smoking. E-cigarettes are not currently approved by the FDA as aids to help stop smoking.

More people began vaping in 2023 than initiated alcohol or illicit substances. A 2015 study suggests that people who vape nicotine are more likely to become dependent on nicotine than people who vape nicotine-free fluids. Vaping might appear to be a less harmful choice for people trying to quit smoking. However, that doesn’t mean there aren’t risks involved, even if the vape liquid is nicotine-free. The study, published on May 23, 2022, in Tobacco Control, is the first to look at the health care costs of e-cigarette use among adults 18 and older. The use of microporous ceramic as a wicking material has increased in the past few years14,16,18,19,20. Its application has been reported to improve heating efficiency and reduce charring14,16,18,19,20. Additional research is ongoing to clarify the dangers from vaping, and how vaping damages the lungs and other organs. Long-term human studies, in particular, could bring the risks from e-cigarettes into better focus, Crotty Alexander says.

An expert independent evidence review published today by Public Health England (PHE) concludes that e-cigarettes are significantly less harmful to health than tobacco and have the potential to help smokers quit smoking.
It would seem that future studies should analyse the possible toxic effects of humectants and related products at concentrations similar to those that e-cigarette vapers are exposed to reach conclusive results.
